Sending a One-Time Assessment to a Client

Last updated on

A one-time assessment lets you ask a client to complete a quality of life assessment for their pet, even if they don’t use Hospet yet. It’s a quick way to see how a pet is doing at home before an appointment, during treatment, or when a family is weighing end-of-life decisions.

Your client gets an email with a link and completes the full assessment in their browser. It takes about 10 minutes and they don’t need a Hospet account. When they submit it, you both receive the results.

📨 Sending an Assessment

  1. Log into your Hospet Connect account and open your Provider Dashboard.
  2. In the One-Time Assessments section, click “Send One-Time Assessment”.
  3. Enter your client’s first name and email address. Their last name is optional.
  4. Optionally, enter the pet’s name, species and breed. Anything you leave blank, your client fills in before they start. They can also correct anything you entered.
  5. Optionally, add a personal note. It’s included in the email to your client.
  6. Click “Send Assessment”. You’ll go back to your One-Time Assessments list, where it shows as Sent.

Your client receives an email from Hospet asking them to complete the assessment. If they reply to it, the reply goes to your practice’s primary contact email.

📊 Getting the Results

When your client submits the assessment:

  • You receive an email with a summary of each category’s score and the full assessment attached as a PDF. It goes to the person who sent the assessment and to your practice’s primary contact email.
  • Your client receives a copy of the results by email.
  • You can view the results in Hospet Connect at any time.

To view results, go to Hospet Connect → One-Time Assessments in the main menu (or “View All” on your dashboard) and click “View Results”. From there you can view or download the PDF.

🔗 When Your Client Creates an Account

After they submit, your client is invited to create a free Hospet account, or to sign in if they already use Hospet. When they do:

  • The pet and the assessment are saved to their account.
  • You’re added to their Support Team, and the pet is shared with you automatically.
  • The pet appears in your Client List, so you can follow their daily ratings, weights and future assessments.

Your client can stop sharing at any time from their Support Team page.

🏷️ Assessment Statuses

Status What it means
Sent Your client hasn’t submitted the assessment yet.
Completed Your client submitted it. You can view the results.
Account created Your client saved the assessment to a Hospet account and shared the pet with you.
Expired Your client didn’t submit it within 30 days.

🔁 Resending an Assessment

Assessment links stay open for 30 days after the most recent email. If your client hasn’t submitted the assessment, click the paper plane icon next to it in your One-Time Assessments list, then confirm. We’ll send them the same email again, and the 30 days start over.

Each assessment can be emailed up to 3 times in total, including the first email.

🗑️ Deleting an Assessment

You can delete an assessment your client hasn’t submitted yet, for example if you sent it to the wrong email address. Click the trash icon next to it in your One-Time Assessments list, then confirm. The link in your client’s email stops working.

Once your client submits an assessment, it can’t be deleted, so the results are never lost.
